Complete these three required forms before June 30. All three forms should be reviewed and signed by both the parent/guardian and the student. Please submit only one form per student. A copy of the completed form will be sent to the email address provided on the form.

This form is not required, but we invite you to complete it if the student attending HSJI will be traveling without a parent/guardian. You may also fill it out if you simply want to keep HSJI staff in the loop.
